Effects of arm cycling on trunk motor function after spinal cord injury

Outline of project

The project aims to study the effect of arm cycling on trunk function following incomplete spinal cord injury (SCI). Evidence has shown that arm cycling can improve walking due to modulations of neural networks in the spinal cord. While it is well established that there are functional interactions between the trunk and limbs, e.g. activating trunk muscles to maintain stability of the body while moving the limbs, it remains largely unknown whether arm cycling exercise can be used to improve trunk function. We hypothesise that a 6-week home-based arm cycling programme can improve neural pathways projecting to the trunk muscles, leading to improved function. The project involves visits to our labs at University of Birmingham where neural and functional assessments will be performed. This allows us to monitor the effect of the intervention on changes in the nervous systems and control of movements. A light-weighted stationary bike will be provided and instructions for installation and exercise at home will be given. Current rehabilitation for improving trunk motor function is complex and requires time-consuming input from both therapists and carers to educate the patients in how to undertake the specific exercises and also to assist the patient whilst they carry them out. These requirements often limit the time in which the rehabilitation can be performed and may therefore impede recovery. Since arm cycling exercise requires minimal training and no supervision, the outcome of this work will significantly benefit people with SCI by offering a novel and affordable approach for trunk rehabilitation.
